A complex bunionectomy, specifically the Lapidus procedure, is a surgical intervention performed to correct a severe or recurrent bunion deformity at the base of the big toe joint, known as hallux valgus. Unlike traditional bunionectomy techniques, which focus primarily on removing the bony prominence, the Lapidus procedure involves realigning the bones of the foot at the base of the first metatarsal, where it meets the midfoot (the cuneiform bone). This procedure aims to address the underlying structural abnormalities contributing to the bunion deformity, such as hypermobility of the first metatarsophalangeal (MTP) joint or a long first metatarsal bone. During the Lapidus procedure, the surgeon makes an incision near the top of the foot to access the first MTP joint and remove a small wedge of bone from the base of the first metatarsal. The joint is then stabilized using screws or plates to maintain proper alignment. By correcting the alignment of the bones and addressing any associated ligament laxity or instability, the Lapidus procedure can provide long-term correction of the bunion deformity and improve overall foot function.
