A Lumbar or Sacral Transforaminal Epidural Steroid Injection (TFESI) is a specialized, minimally invasive procedure designed to alleviate pain and inflammation in the lower back (lumbar region) and the sacral area. This procedure involves the precise delivery of a corticosteroid medication, often combined with a local anesthetic, directly into the epidural space of the spine through the foramina—the openings where nerve roots exit the spinal column. The goal of TFESI is to reduce inflammation and pressure on the affected nerve roots, providing relief from conditions such as herniated discs, spinal stenosis, degenerative disc disease, and sciatica, which is characterized by nerve pain radiating from the lower back down into the legs.| In the lumbar region, which includes the five lumbar vertebrae (L1-L5), TFESI addresses pain originating in the lower back and radiating into the hips, buttocks, and legs. The sacral region, located just below the lumbar spine and comprising the sacral vertebrae (S1-S5), is targeted to alleviate pain in the lower back and pelvic area, extending potentially into the legs. During the procedure, the patient lies face down, and the injection site is thoroughly cleaned and numbed with a local anesthetic. Using fluoroscopic (X-ray) guidance, the physician inserts a thin needle through the skin and soft tissues to the transforaminal space adjacent to the affected nerve root.
