A Cervical or Thoracic Transforaminal Epidural Steroid Injection (TFESI) is a specialized, minimally invasive procedure aimed at alleviating pain and inflammation in the cervical (neck) and thoracic (mid-back) regions of the spine. This technique involves the precise administration of a corticosteroid medication, often combined with a local anesthetic, into the epidural space through the foramina—the openings where nerve roots exit the spinal column. This targeted approach helps reduce inflammation and pressure on the affected nerve roots, providing relief from conditions such as herniated discs, spinal stenosis, degenerative disc disease, and radiculopathy, which is characterized by nerve pain radiating from the spine to other parts of the body, such as the arms or torso.| The procedure varies slightly depending on the spinal level being treated. In the cervical region, which includes the seven cervical vertebrae (C1-C7), TFESI is used to address pain originating in the neck and radiating into the shoulders and arms. For the thoracic region, encompassing the twelve thoracic vertebrae (T1-T12), the injection targets mid-back pain that can extend around the rib cage and into the chest or abdomen. Regardless of the level, the patient lies face down, and the injection site is thoroughly cleaned and numbed. A contrast dye is typically injected first to confirm accurate needle placement before administering the steroid medication.
