Surgical treatment of an anal fistula involves various techniques aimed at eliminating the abnormal connection between the anal canal and the skin while preserving anal sphincter function to prevent incontinence. The most common procedures include fistulotomy, where the fistulous tract is surgically opened to heal from the inside out; seton placement, which involves inserting a surgical thread to keep the tract open for gradual healing and drainage; and the advancement flap procedure, where the internal opening of the fistula is covered with a tissue flap to promote healing. Another technique, the Ligation of Intersphincteric Fistula Tract (LIFT), involves ligating and dividing the fistula tract to separate it from the anal sphincter. Additionally, fibrin glue or a collagen plug may be used to close and heal the fistula tract. The choice of procedure depends on the fistula's complexity and its relationship to the anal sphincter muscles. The primary goals are to ensure complete healing, prevent recurrence, and maintain continence.
